Clarity Call
Take advantage of a free clarity call! Many clients use this option to ask more about our specific functional medicine programs or services that we offer. We are able to answer any questions you may have about how the process works and tell us more about your wellness goals.
In addition, we want to make sure that we’re a good match and are able to provide the holistic care you are looking for. If it seems like a good fit, then we set up the next available appointment to get started.
Prior to your First Consultation
Once we get you registered in the HIPPA compliant charting system, you will receive via digital consent forms, along with a nutrition, lifestyle, & review of body systems questionnaire. We ask that you complete these forms prior to your first consultation. This gives us time to look over your past health history in detail. If you have recent lab work (within 1 year) that you would like to share with us we have an upload option within our charting system, otherwise, bring them to your first consult.
In-Person | Virtual Visits
We have many options available to you for any of your visits with us. If you are local, we offer in-person visits during business hours. If virtual consultations works better for you, we offer HIPPA compliant video chat, Zoom, FaceTime, or Telephone options. We will provide you with a secure link to any video chat or Zoom meeting the day before we are to meet.

The Initial Consult
We will spend approximately 90-120 minutes reviewing your current health concerns and discussing your past history. It is helpful during this first meeting to have information about your current medications, diagnosis, and any supplements you are currently taking. We use this initial consult to establish a framework for the subsequent consultations.
Functional Health Testing
Based on our initial consult and specific health concerns we will discuss options for functional lab testing that may help guide our nutrition and holistic treatment plan.
Depending on the labs – there may be on option for billable by insurance and should be done at your discretion. The easiest way to find this out is to call your insurance company to see what labs may be covered.
Many of the labs we utilize have been negotiated at a discounted rate and are included in our program | holistic package that we offer. This means, no extra fees will occur once we get started with your healing process unless we discuss them with your first.

Follow-Up | Consultations
Our follow up meetings will look different from the first initial consult. Typically, we spend about 30-45 minutes to discuss follow-up results, nutrition, supplement, herbal, and treatment plans adjustments throughout your wellness journey.
We will work in a step-by-step approach to develop fundamental healing during the first month and refine our holistic plan based on your health improvements. Expect an actionable plan following each visit.
Payment
We do not accept insurance for any of our services offering at Pure Root Wellness, but we are eligible to accept HSA & FSA. In addition, we are able to provide you with a SuperBill that you can submit into your insurance for possible reimbursement. The qualification and amount of reimbursement is dependent on your insurance policy and their coverage of out-of-network providers.
Payment for services are required before or at your first consultation. Payment in-full or payment plan options are available and will be determined prior to our first consultation.
Financing options are also available through our partnership with CHERRY, a third party credit services are designed specifically for medical services and offer payment plan options to cater to your payment needs. They offer 6 month to 48 month payment plans. Payment Plan Options
